Artist Member Application Form

Please mail the above application with the following materials:
1. Images of five (5) artworks - two views are required of all sculpture. Either 35mm slides or digital images are acceptable (see below for instructions)
2. $40.00 jury fee – check or money order is accepted
3. Self-addressed stamped envelope for return of slides - Slides will NOT be returned without SASE. CDs WILL NOT BE RETURNED.
- For return of slides to address outside of U.S., please include additional $2.00 USD (jury fee = $42.00 USD)
Instructions for Digital Images:
• Digital images must be sent via postal mail on a PC-compatible CD.
• NO EMAILS WILL BE ACCEPTED!
• Images must be 300 dpi at 8" wide and in either JPEG or TIFF format.
• CDs should be labeled with the artist's last name and first name. Be sure to list the images by title in the order they appear on a separate inventory sheet. Include subject matter, medium and image size on sheet.
Instructions for Slide Preparation:
Slide Tips
• Flatwork should extend to edges of slide - framing or backdrop should not be visible
• Check that slides are crisply focused and neither too light nor too dark
• Sculpture must be represented by two views
• Glass mounted slides are not acceptable
• Please use ball-point or permanent pen when labeling slides - pencil and fountain pens smear and can damage slides
• Please indicate with an arrow and the words UP or TOP which way slide should be oriented for correct viewing
